How to get in network with insurance in California: payer-by-payer guides for therapists

Every insurance company has its own portal, its own application, and its own behavioral health company behind the card. These guides walk through each one for California LMFTs, LCSWs, LPCCs, and psychologists, grouped by the portal you will actually use.

Short answer

Most California commercial plans credential therapists through a behavioral health administrator or portal: Optum (Provider Express) for UnitedHealthcare and Western Health Advantage, Evernorth for Cigna, Availity for Anthem, Aetna’s own request form, Carelon for Kaiser and several other plans, and Blue Shield of California directly. Start with an NPI, a complete CAQH profile, malpractice coverage, and a W-9, then apply payer by payer. Networks can be closed in some areas, acceptance isn’t guaranteed, and timelines vary.

Before you apply anywhere

The same foundation works for every payer: an individual NPI, a complete and attested CAQH profile (now branded DataSpring, same login), current malpractice coverage, a signed W-9 that matches your tax ID, and a clear list of practice locations. My insurance credentialing guide covers the general process, and Headway vs. Alma vs. direct contracts helps you decide whether to apply directly at all.

Associates: payers don’t credential AMFTs, ASWs, or APCCs as individual providers, but several let the employing practice bill for supervised associate sessions. Optum (UnitedHealthcare), Aetna, Cigna/Evernorth, Blue Shield of California, and Medi-Cal fee-for-service publish rules for it, and each wants the claim built differently. Medicare and TRICARE don’t pay for associate services, and Anthem and Carelon have no clear public rule. Associate billing must be confirmed in writing with each payer; see can associates bill insurance in California for the payer-by-payer table and sources.

Optum / Provider Express

Optum Behavioral Health credentials and contracts for UnitedHealthcare, and it is the behavioral health partner for Western Health Advantage. One application on Provider Express covers this family.

Availity

Anthem and Carelon take network applications inside Availity. Aetna takes its request to join on its own site, then uses Availity for day-to-day work. TriWest (TRICARE) also uses Availity as its provider portal.

Evernorth

Cigna behavioral health is Evernorth Behavioral Health. You apply in the Evernorth Provider Portal.

Carelon

Carelon Behavioral Health manages behavioral health networks for several California plans. New enrollment goes through the Carelon payer space in Availity; Kaiser referrals use a separate Carelon portal.

Medi-Cal / PAVE

Medi-Cal plans require Medi-Cal enrollment (or proof of a PAVE application) before or alongside plan credentialing.

Other

Plans that run their own process or a separate program.

Business education only, not legal, billing, or clinical advice. Plans change administrators and portals; confirm current rules with each plan before you apply. Bill only for what you legitimately provide and document.

Starting a new practice? A platform like Headway credentials clinicians with several commercial plans under its own agreements, so it can be a way to start seeing insurance clients while direct applications are pending. Check which plans it offers in your area. If that fits, you can join Headway through my referral link. Alma and contracting directly with the plan are also good options.
