How to get in-network as a therapist (and whether Medi-Cal is worth it)

Insurance is where a lot of therapists get stuck. The paperwork is confusing, the timelines are slow, and nobody explains the difference between "we applied" and "we can bill."
Here's the landscape in plain language.

What therapists usually worry about with insurance

  • "Should I just use Headway or Alma?"
  • "How do I get in-network on my own?"
  • "Is Medi-Cal worth the paperwork?"
  • "Why has my application been 'pending' for months?"
  • "What if I bill wrong and get audited?"

Your three paths

PathProsCons
Cash-payFull control. No claims.Smaller client pool. Harder in some areas.
Platform (Headway / Alma)Fast credentialing. The platform handles billing.Platform sets the rates. Less ownership. Harder to add associates.
Direct contracts (commercial + Medi-Cal)You own the contracts. Scalable with associates. Medi-Cal brings steady referrals.Paperwork-heavy. Slow. Needs internal systems.

There's no single right answer. Many practices combine paths, for example cash-pay plus one platform, or direct commercial contracts plus Medi-Cal.

Getting in-network on your own

CAQH

CAQH ProView is the credentialing profile most commercial payers pull from. Keep it complete, attested, and up to date. An expired attestation can stall applications.

PAVE

PAVE is California's online portal for enrolling as a Medi-Cal provider. Medi-Cal enrollment is its own process, separate from commercial credentialing.

Medi-Cal managed care plans vs. county specialty mental health

Medi-Cal mental health services are generally split between managed care plans (which generally cover mild-to-moderate mental health services) and county specialty mental health plans (which generally serve more severe needs). Who you contract with shapes which clients you see, how referrals come in, and what documentation is required. Requirements vary by county and plan.

Credentialing vs. contracting vs. effective dates

These are three different things:

  • Credentialing — the payer verifies your license, education, and history.
  • Contracting — you sign an agreement that sets your rates and terms.
  • Effective date — the date you can actually bill as in-network.

You can be credentialed without a contract. You can have a contract without knowing your effective date. Don't see clients as in-network until you have the effective date in writing.

Why "we applied" isn't a status

"We applied" tells you nothing. A real status sounds like: "Credentialing approved on [date], contract signed on [date], effective [date], rendering provider ID [number]." Track every application with dates, reference numbers, and who you spoke to.

Medi-Cal economics

Medi-Cal often gets written off as "charity work." It doesn't have to be.
Example (labeled — not a rate quote):

Example of a properly documented Medi-Cal session at one California plan's rates: psychotherapy $131.97, telehealth add-on $14.40, PHQ-9 screening $4.81, plus case management when medically necessary and documented. Rates vary by plan, county, and date. Verify your current fee schedule.

Medi-Cal is not charity work when your systems capture everything you legitimately provide and document. That means accurate documentation, correct codes for what you actually did, and a claims process that doesn't let legitimate services slip through.
Bill only for what you legitimately provide and document.

Common expensive mistakes

  • Seeing clients before your effective date and getting denied claims.
  • Letting your CAQH attestation lapse.
  • Treating "applied" as "approved."
  • Not tracking applications with dates and reference numbers.
  • Underdocumenting services you legitimately provided, so they never get billed.
  • Signing a contract without reading the rates and terms.
