How to Get In Network With UnitedHealthcare in California (as a Therapist)
Clients keep showing up with a UnitedHealthcare card, so you go looking for the UnitedHealthcare application. You won’t find a separate one for therapists. Here’s why, and what to do instead.
UnitedHealthcare doesn’t credential therapists itself. UHC says Optum Behavioral Health Solutions handles credentialing and contracting for behavioral health on its behalf, so California therapists apply through Optum’s Provider Express portal (or call 800-817-4705). An Optum agreement is how you become in network for UHC commercial members’ mental health benefits. UHC Medicare Advantage is a separate line: confirm which networks your agreement covers and whether Medicare enrollment is required. Acceptance isn’t guaranteed and timelines vary.
The one thing to know: UHC sends therapists to Optum
UnitedHealthcare’s own join-our-network page says that for behavioral health, you contact Optum Behavioral Health Solutions, which handles credentialing and contracting on behalf of UnitedHealthcare, starting at providerexpress.com or 800-817-4705. So the application is the same one described in my Optum guide. This page covers what’s different when you start from the UnitedHealthcare side: which California plans you’ll see, how to read the card, and the commercial vs. Medicare question.
UnitedHealthcare plan types you will see in California
UHC lists a long menu of California commercial plans. The names matter because they tell you how the member’s benefits work:
| Plan family | How it works (per UHC) |
|---|---|
| Choice / Choice Plus | Open access, no referral. Choice Plus adds out-of-network benefits at a lower level. |
| Options PPO | PPO with in-network and lower out-of-network benefits. |
| Select / Select Plus, Core / Core Essential | Open access; some networks are narrowed in some states. |
| Navigate, NexusACO | PCP-coordinated; some versions require referrals for network specialists. |
| SignatureValue (HMO family: Advantage, Alliance, Focus, Flex, Harmony) | California HMO plans built around a PCP and medical groups. |
| Surest | UHC says Surest members can use the UnitedHealthcare and Optum Behavioral Health networks, and participating UHC commercial providers automatically participate in Surest. |
For mental health, the member’s behavioral health benefit is what matters, and UHC’s behavioral health network is Optum’s. Before the first session, verify benefits for the specific plan. Some plans have narrower networks or referral rules that the plan name alone won’t tell you.
Employer plans: fully insured vs. self-funded
Most UHC members in California get coverage through an employer. Some employers buy a fully insured UHC plan; others are self-funded, with UHC administering the plan. You don’t need to know which before you apply, but it explains why two clients with “UnitedHealthcare” on the card can have different deductibles, copays, and authorization rules. Always check the specific member’s benefits.
How to read the UHC member ID card
The card tells you where behavioral health goes. Look for:
- A behavioral health or mental health phone number on the back of the card. On many UHC plans it routes to Optum.
- The plan name and group number, so you can match the plan family above.
- The claims payer ID and address. Use exactly what the card and Optum’s instructions say for behavioral health claims. Don’t assume the medical claims address is right for therapy claims.
If anything on the card is unclear, verify eligibility and benefits through Provider Express once you’re contracted, or call the number on the card.
UHC commercial vs. UHC Medicare Advantage
These are different lines of business, and therapists mix them up all the time:
- Commercial (employer and individual plans): this is what most private practice therapists mean by “taking UnitedHealthcare,” and it is what the Optum application covers.
- Medicare Advantage (including AARP-branded plans): these are Medicare plans run by UHC. Since January 1, 2024, LMFTs and mental health counselors can enroll in Medicare and bill it independently, which opened this door for California LMFTs and LPCCs for the first time. Optum’s Provider Express refers to separate commercial and Medicare Advantage networks, so ask Optum which networks your agreement includes and whether Medicare enrollment is required for the Medicare Advantage network.
Step-by-step: getting in network for UHC members
- Get your foundation ready: independent California license, individual NPI, current malpractice, a signed W-9, and a complete, attested CAQH profile authorized for United Behavioral Health/US Behavioral Health Plan.
- Register on Provider Express, choose California, and go to Our Network.
- Apply as an Individually Credentialed Clinician if you are solo or your group has no Optum group agreement. Otherwise use the group path.
- Make CAQH and your application match exactly. Optum lists mismatched locations and missing authorization as top causes of delays.
- Return DocuSign requests quickly. Optum sends contract and disclosure documents that way.
- Track status under My Practice Info, My Network Status, Check Initial Credentialing Status.
- Wait for your effective date before treating any UHC member as in network.
The full walkthrough, including what to attach, is in my Optum guide.
When you are approved
Once your agreement is effective, UHC commercial members whose behavioral health runs through Optum can find you in the directories. Verify each client’s benefits, check whether the plan requires authorization for the service, and submit claims the way Optum instructs (Optum says outpatient claims go electronically through Provider Express or EDI). Bill only for what you legitimately provide and document.
Associates and other routes
Associates: UnitedHealthcare behavioral health runs through Optum. Optum doesn’t credential associates individually, but its commercial supervised-services policy (2024RP202A) covers California, in and out of network. Claims need the U5 modifier on each supervised line and must identify the Optum-credentialed supervisor (Optum policy). It does not apply to UnitedHealthcare Medicare Advantage plans. See Can associates bill insurance in California?.
Headway and Alma: Headway says its clinicians accept UnitedHealthcare and all Optum affiliates, and its team handles credentialing under Headway’s own payer agreements. If you would rather go that way, here is my Headway referral link.
Referral disclosure: I may receive a referral reward if you sign up through my Headway link. It costs you nothing extra, and you get the same help from me either way.
Alma is another option for some plans, and direct contracting through Optum gives you your own agreement. See Headway vs. Alma vs. direct insurance.
Rules change. Everything here is general business education for California therapists as of October 2026, not legal, tax, or billing advice. Networks open and close, acceptance is never guaranteed, and timelines vary. Confirm current requirements with the plan (in writing) before you act.
Referral disclosure: I may receive a referral reward if you sign up through my Headway link. It costs you nothing extra, and you get the same help from me either way. Alma and contracting directly with the plan are also good options.
Frequently asked questions
Does UnitedHealthcare have its own application for therapists?
No. UHC says Optum Behavioral Health Solutions handles behavioral health credentialing and contracting on its behalf. You apply through Provider Express or call 800-817-4705.
If I am in network with Optum, am I in network with UnitedHealthcare?
For UHC members whose behavioral health benefits Optum manages, an Optum agreement is how you are in network. Confirm with Optum which plans and lines of business your agreement covers.
Can I see UHC Medicare Advantage clients as an LMFT?
Possibly. Since January 1, 2024, LMFTs and mental health counselors can enroll in Medicare. Ask Optum whether your agreement includes its Medicare Advantage network and what enrollment it requires.
What is UnitedHealthcare SignatureValue?
UHC’s California HMO plan family (including Advantage, Alliance, Focus, Flex, and Harmony versions), built around a PCP and medical groups. Verify the member’s behavioral health benefits and any referral rules before the first session.
Can associates bill UnitedHealthcare?
Not in their own name. For UnitedHealthcare commercial plans in California, Optum’s supervised-services policy lets the employing practice bill associate sessions with the U5 modifier and the Optum-credentialed supervisor on the claim. Medicare Advantage is excluded. See Can associates bill insurance in California?.
